RESEARCH DIAGNOSTIC CONFIRMATION

Confirming your ASD Diagnosis

We do not usually have an opportunity to meet our research participants in person. Therefore, we strongly encourage a confirmation of an ASD diagnosis for the individual(s) with an ASD. Without a diagnostic confirmation, we may not be able to include your results in our study as they validate our data by ensuring that all our participants do have a diagnosis of an ASD. Ideally, we would like to receive a copy of a diagnostic report or letter from a medical professional (by fax, email or by mail; all identifying information will be removed after it is received). Alternatively, if you don't have a report or prefer not to share one, you can complete the PDDBI questionnaire.

• Fax: toll-free: 1-888-556-60571-888-556-6057 or 613-548-8385613-548-8385
• Mail: Autism Research Program, 191 Portsmouth Avenue, Kingston, ON, Canada, K7M 8A6 (We can send you a pre-paid postage envelope for within North America)
• Email: autism.research@queensu.ca (The documents can be scanned and password protected, then send the password in a separate email.)

Please note that the information in all diagnostic reports will remain strictly confidential and the reports are stored in locked cabinets within a secure area. We will email you to confirm that we have received your report. If for any reason you have not received an email confirming that we received the report, please follow-up with us.
